entities as nodes

Hi there-
its my view that the data model needs to be changed to represent
entity-references as nodes:
(a) this would allow conditional processing of entities
(b) this would allow xslt processors to 'pass through' entities without
having to explicitly define each one (which is crucial when you're dealing
with large numbers of entities, for example, ISO entity-sets)
(c) this would simplify the generation of entities in xml output.
