top-level elements, AVTs, and xsl:output

If the attributes of the xsl:output element are going to be treated as
attribute value templates, then the following part of 7.6.2 needs to
be updated: "Attributes whose value is an expression or pattern,
attributes of top-level elements and attributes that refer to named
XSLT objects are not interpreted as attribute value templates."
