XInclude 1.1 support in XML Calabash

In the next release of XML Calabash, expected before XML Prague ;-),
support for XInclude 1.1 will be against the 16 December 2014 draft,
http://www.w3.org/TR/2014/WD-xinclude-11-20141216/

The relevant changes:

* Attributes in the xml: namespace are no longer copied
* You can specify an xml:id value with set-xml-id
* You can assign values to attributes in no namespace by
  putting them in the "local-attributes" namespace on the
  xi:include element.
