New release of XSV sources and EXE (was Re: key/keyref problem)

I've fixed the "identity constraints are qualified names" bug, and
updated the executable as well as the sources.  See the status page [1]
for details.

Note this fix may cause some previously working-but-actually-broken
schema documents to (correctly) show errors now.
