problems with html markup in REC-xml-names

Folks,

I realized a bit late that the xml namespaces REC [1] had invalid html.
I fixed it:

1) I mentioned during an earlier document publication that the
xml-to-html convertor that you used is confused about some types of
nested tags (mostly <code><a href...></a></code>: it swaps the end tags
for some reason...). It is also confused about <p> in <span> or <span>
in <p>.
2) the TOC was marked up using &nbsp; for indentation (!). I replaced
the markup with nested definition lists (<dl><dd>...</dl>).
3) the mailto: to xml-names-editor was broken
4) I made the reference to the XML REC a clickable one.

I fixed 3) and 4) in the xml version too.

best regards,

 - Renaud
