nit: spaces in quotes

In the HTML version of the the review copy, there are spaces inside quotes
where they are inappropriate. The problem comes from SGML record-end
handling being applied to the file; there are cases of

"<code>&amp;gt;
</code>"

which HTML browsers interpret as "&gt; " - not what was intended. The
comment at the top of the review copy indicates that someone edited the
HTML with ADEPT, which may have re-wrapped the line ends in the file.
