a) Productions 49 and 50 have instances of "cp" without underlines.  Does
this mean something or is it an oversight?

b) Should the first asterisk in production 51 be a plus sign?  
As it is, the following are both legal instances of Mixed:
	(#PCDATA)*
	(#PCDATA)
If these are indeed supposed to be legal, the two lines of production 51
can be combined into the first by replacing the trailing " ')*' " with
	')' '*'?
