Updated Simplified req-resp MEP with soap-response mep

I've attached Simple MEP proposal with the soap-response MEP.

 

I've updated this proposal to use some of the wording that Noah
suggested in his proposal in the HTTP Binding.  Where I disagreed with
his wording, I kept his wording to keep these synchronized and sent
separate comments to the list.
