This looks great; thanks Stuart.

If we go down this path, it would be interesting to explore and
formalize the relationship between 'correlation', 'causality' and
'message exchange pattern'. At first glance, it sounds like there's a
correlation (sorry) between correlation and Causality.MessageRef. 

Is it the case that the messageref (correlation) and sequence
together define not only causality but also the XMLP (not transport)
message exchange pattern?
