For some reason such an element declaration was not in the SOAP 1.1 Schema.
When we produced the schema for SOAP 1.2 we converted the SOAP 1.1 schema to
confrom to the XML Schema recommendation. Because we just did a conversion
we don't have an element decl for fault in the SOAP 1.2 schema either.

Another commentator also bought this to my attention and I agree that we
should add such an element decl.
