Value space of QName

Part 2 describes the value space of QName as "The value space of QName
is the set of tuples {namespace name, local part}, where namespace name
is a uriReference and local part is an NCName".  This is not quite
right: a QName need not have a prefix; in such a case, the namespace
name part may be missing.
