minor errors in part 0: primer

In '2.6 Annotations' of the XML schema part 0 draft (7 april) there is an
example used to show the use of annotation. The 'annotation' and
'documentation' elements should have the prefix 'xsd:', since that is the
prefix associated with the XML Schema namespace.
