I believe that the unnamed intermediate case where the destination anchor
replaces the source *anchor*, not the source *document* should be named and
defined.

This would allow documents to be tailored to meet the requirements of
different audiences by substitution of a source anchor with a destination
anchor.

The other facility which is missing is the ability to delete a source
anchor.
