The sentence:

"It allows up to five attributes, each of whose value shall be a URI
reference [[RFC 2396]]."

should read:

It allows up to four attributes, each of whose value shall be a URI
reference [[RFC 2396]]. 

There are four possible attributs of the fcs element, not five.
