Newline mapping in XML 1.1

In XML 1.0, we require newline mapping only for what the app gets, i.e.
character content/attribute values.  The definition of S maps all
newlines in markup into whitespace.

In 1.1, we need to extend the rules to do newline mapping on the whole
document, since S will not contain #x85 or #x2028.

The Core WG has already agreed to this, and this email is for tracking.
