updated spec with new schema

Folks,
 
I have updated the specification with new schema.  I will update the spec with the samples in a day or two.  One noteable todo item is to scrub mention of QName thro' out the spec especially when describing the corresponding schema.  The change log is at the bottom of the spec.

Also, I updated the annotation part in the xkms.xsd.  This new schema is available @
