New draft and schema

All,

	Attached is my current editors draft of XKMS. I have attempted to
capture as many issues as possible in the document (see the end of the doc
for a comprehensive list).

	This is still a work in progress, I have made significant changes to
improve the consistency of the doc, the schema has also been changed
somewhat to improve consistency, remove obolete elements etc.
