Cleanup of spec.

This is obvious, but I wanted it to be captured.
The examples in the spec have not been updated to be consistent
with the schema (i.e. the schema in the Key Information Service and
Key Registration Service Message Set).

Also, these Message Sets still have remnants from previous drafts which need to be updated.
For example, for the LocateRequest element, there is no longer a Query or Respond element,
but descriptions for Query and Respond still appear under the LocateRequest declaration,
and similarly for the LocateResponse and all of the other Request and Response elements
(for Validate, Register, etc.)
