Updated Key Management Requirements draft

Here is the update to the XML Key Management Requirements draft that
(hopefully) takes into account the email comments from Blair, Ed, Rich,
Sébastien, Stephen and Yassir, as well as any decisions from last week's
teleconference call. 

Most new text that was added is underlined, and the major changes have
hyperlinks to the email requesting the change. 

This will be the version presented and discussed at our meeting on December
9th. However, comments send to the list before then would be greatly
appreciated.
