Update to Patterns (was MEPs)

I have updated the WSDL 1.2 Patterns doc to reflect decisions made at
the last face-to-face meeting. Notably, it includes 'friendly' names for
the patterns and the request-response pattern that constraints out
messages to be sent along the same channel.
