Issues Doc revised

Dan C. pointed out that I was using IDs that were syntactically
non-conformant with XHTML.  Fixed.  

Note that this will mess up existing pointers into this document.  The
change was to insert an 'I' at the front of each reference, since IDs may
not begin with a digit.
