CCXML events

Section 9.4.3 describes several standard events.  For example, "ccxml.joined" if a successful join happens.  But I can't seem to find any standard events for certian other things, for example:
 
1) When <hold> is performed, what events are generated for failure and success?
2) What if a <join> fails?
3) What events happen after a <createconference>?
