This strikes me as pure sillyness; a convoluted mess of interacting rules
from different parts of the prose that result in a completely illogical set
of legal combinations. Valid HTML that is labelled Invalid by the Validator
is a Very Very Bad Thing.

I would suggest a note to the effect that "This makes no sense whatsoever;
don't worry about it or you'll get a migraine" in relevant parts of the
spec; if it isn't possible to disambiguate the rules in a proper fashion.
