[CSS21] background-attachment: fixed not defined for paged media

Fixed positioning is defined in
   http://www.w3.org/TR/CSS21/visuren.html#fixed-positioning
but the positioning and painting of fixed background images do not seem
to be defined anywhere for paged media.
