This is pretty academic. For the two stylesheets on the planet which will
ever meet this case out of the six billion existing documents and all
those to come in the next few decades, I suggest we simply leave it up to
the UA. Or do what Bert suggested, use UTF-8. It doesn't matter anyway as
it is unlikely that those docments will even contain surrogates. Nor will
they even be sent over the Web since CESU-8 is not an over-the-wire
format. Could we please focus on cases that will actually happen? :-)
