How to represent metadata in/alongside documents

What is the current thinking on how to represent metadata associated with a
document - Given three cases 'myDoc.doc', 'myDof.pdf' or 'myDoc.html', Do
you put some rdf with dublin core metadata in a separate file 'myDoc.rdf'
Any examples of usage and formats this way or otherwise?
