new (**in progress**) issues list

QAWG --

We closed 31 issues at Tokyo!

I figured it was better to post this in-progress issues list [1] for the 
teleconference, even though all resolutions are not completely documented 
yet.  The issues are correctly marked closed, but some have "@@to be 
written@@" for the disposition.  (I have the information in hand, but not 
enough time -- also, want to see final minutes before calling it all done.)
