[MathML2] MathML elements appendix confusing

Hi,

In http://www.w3.org/TR/MathML2/appendixl.html several element names are  
mentioned that are never defined and appear to be actually only attributes  
in MathML2. Examples include:

   function
   display
   style

There are probably more. I think it affects all names that don't have any  
link with the class of "mainindex" in their description.
