Let's note that early MathML application developers were shooting at a
moving target.

Different strategies among and different implementations by browser
developers are a part of the scene.

In fact, that we have a working cross-platform technique is something
short of a miracle.  (Thanks to David Carlisle for it.)

The current guidelines for cross-platform interoperability were
"released" at MathML 2002 (the end of June).  See

That is rather recent after all.
