The xhtml + MathML DTD as included in the mathm2 REC is based on a draft of XHTML 1.1 supplied to us by the HTML working group. MathML 2 was completed before XHTML 1.1 so the final version was not available then. There is a new _draft_ version currently under construction at http://www.w3.org/Math/DTD with some additional documentation at http://www.w3.org/Math/characters the version there uses the driver file http://www.w3.org/TR/XHTMLplusMathMLplusSVG/ so, the version http://www.w3.org/Math/DTD/mathml2/xhtml-math11-f.dtd is based on the published version of XHTML 1.1 and hopefully you will not have problems with this version (let us know if you do!)
