Modified references to East Asian width

https://github.com/w3c/charmod-norm/issues/19

Removed the explicit reference to East Asian width from the table, using the suggested terminology 'width variation' to match UAX15.

Modified description text to be more generic. Added a reference to UAX11 (since nearly all width variations have their origin in East Asian width properties and legacy encodings: best not to ignore that) and also to UTR50 (which explains vertical presentation forms).
