There are a few errors in XHTML's Character Entities!

<!ENTITY amp     "&#38;#38;"> <!--  ampersand, U+0026 ISOnum -->
<!ENTITY lt      "&#38;#60;"> <!--  less-than sign, U+003C ISOnum -->
in XHTML 1.0

<!ENTITY lt      "&#38;&#60;" ><!-- less-than sign, U+003C ISOnum -->
<!ENTITY amp     "&#38;&#38;" ><!-- ampersand, U+0026 ISOnum -->
in XHTML modularization

They should be compliant to HTML 4.01
<!ENTITY amp     CDATA "&#38;"   -- ampersand, U+0026 ISOnum -->
<!ENTITY lt      CDATA "&#60;"   -- less-than sign, U+003C ISOnum -->
