table of contents

I cannot find any provision in HTML 4 for
the automatic generation of a table of
contents (i.e., a nested list---with controllable
depth---of the contents of the header elements).
Is there such? (I've been told it exists in SGML.)

If not, how is this basic component of
document preparation supposed to be
handled?
