following two lines:

<html xmlns="http://www.w3.org/1999/xhtml" xml:lang="en">


It seems to me that the second line is missing 'lang="en"'. I think it
should be

<html xmlns="http://www.w3.org/1999/xhtml" lang="en" xml:lang="en">
