I have been puzzling over the following lines from the (HTML40) DTDs:

    <!ELEMENT BODY O O (%flow;)* +(INS|DEL) -- document body -->
     <!ELEMENT HEAD O O (%head.content;) +(%head.misc;) -- document head
-->

Either those + signs are in error, or -- and that's more likely -- the
description in section 3.3.3 is incomplete. In that case a short
paragraph should be added to the end of 3.3.3. Saying: "Similarly, the
+(A) construct explicitly says that A _can_ appear in the defined
element. See for example HEAD and BODY."
