The text of <http://www.w3.org/TR/WD-html40/interact/forms.html#h-9.1.1> 
describes an ACCEPT attribute on the FORM element, but the DTD (including 
the DTD fragment at that URL) does not have an ACCEPT attribute, and one 
wouldn't make a lot of sense with an ACCEPT attribute already on the INPUT 
element.
