Many of the links on that website are broken (e.g.

Also, the page:

http://www.eb-forms.com/index.php?option=com_content&task=view&id=15&Itemid=36

mentions a download zip file, but provides no link to it.

By trial-and-error, I found:

http://code.google.com/p/eb-forms/downloads/list

but that looked like it just contained .pdf files.

IMO, that website needs a lot of work.
