Should be moved to section 4.5 Error Indications

In the sentence: "Dispatched by the processor immediately after the
first failure of an output to render or update the rendition of
content."

What does "first" mean here? The first failure of any output? The
first failure of each output? The first failure of each invocation of
an output?

It is not clear here what causes such a failure. Please link to 8.1.5
