Ref: http://www.w3.org/TR/xforms/slice4.html#evt-reset

"The instance data is reset to the tree structure and values it had
immediately after having processed the xforms-initialize-done event"
Once the instance data is reset, some of the inserted nodes will disappear
and some deleted nodes will reappear. Wouldn't this require
xforms-form-control-initialize before dispatching xforms-rebuild,
xforms-recalculate, xforms-revalidate and xforms-refresh?

Regards,
Shailesh Karandikar
