Deferred update and multiple models

Hi,

I think the deferred update behavior described in chapter 10.1.1 needs a 
minor change: there should be a set of flags per <model>. For instance, 
not all models should have their refresh flag cleared if the following 
action is used: <xform:refresh model="model1"/>.
