DOM L3 LS: Using PI to represent XML declaration

DOM L3 LS should either prohibit creating a processing instruction node to represent an XML declaration (as is done by MSXML and Oracle's XML parser) or there should be a configuration property and the default value should suppress creation of a PI (and the alternative value should not force the creation of a PI).
