The following heading in the "table of content" (TOC)

> D.3.1 If possible, don't reuse existing vocabularies 

should be

"D.3.1 If possible, reuse existing vocabularies"

Also, the TOC contains a number of superfluous "li" 
elements after items 2. and 4.1.1.2
