Summary:

   1. PFWG's first comment was responded to promptly. It was not recorded
      in the first DoC due to submission after CR was published.
   2. The first comment was not recorded in the second DoC either because I
      had attempted to address all open issues before publishing LC, and
      therefore did not list any comments prior to the LC publication date
      when compiling the DoC.
   2. PFWG's second comment was addressed prompty, but was not recorded in
      the LC issues list due to my oversight.
   3. PFWG did not acknowledge any of my responses or respond to any
      questions therein.
