Editor's TODO list

I've updated the TODO list[1] with entries for some of the editorial
issues (256, 271, 278, 281, 283, 283, 284, 285, 286, 288 and 289).
Please take a look at these and be sure you are happy with my proposed
resolution. Some of the proposed resolutions are actually questions, I
need input from all the editors on these.

I've also sent mail to you all asking for input on 253, 279 and 280.
Once I have your input on these I will add them to the edtodo. I will
continue adding the remaining editorial issues to the edtodo tomorrow.
