early ACSS note discussing user vs. author control

This is old, but I want it in the file to document the history of
discussion of user vs. author control in the evolution of access provisions
in W3C pronouncements.

The attached note was initially written as a discussion of styling features
for audio styling, but that review provoked a discussion of the general
approach.
