UK links are out of date

Hello,

I was perusing the list of accessibility related policies for the UK in your
web page on this topic (http://www.w3.org/WAI/Policy/#UK) and found some of
the URLs are out of date.


   - Disability Rights Commission
   (it seems this is now the Equality and Human Rights Commission -
   http://www.equalityhumanrights.com/)

   - List of web guidelines related publications
   (the only similar item I found was this archive:
   http://archive.cabinetoffice.gov.uk/e-government/resources/handbook/introduction.asp
   )
   It seems the COI is now charged with the UK web guidance:
   http://www.coi.gov.uk/guidance.php?page=188.
