New test suite evaluation form

I have updated the UA test suite evaluation form[1]. It now submits to 
a PHP script to generate the XML, rather than relying on JavaScript. It 
should be backward-compatible with existing evaluation documents. 
Please feel free to use this form for your evaluations, and email me 
with any questions, comments or issues.
