Use <hr /> for the one at the top and bottom of the pages. Use CSS to make
it look as you wish.

For the ones in the table use either <hr /> or better still use CSS on the
table (td{border-bottom: 1px solid #CCCCCC;} or similar) and get rid of the
cells with the image altogether.
