Issue #3 Programmatic Notification

I've tried to address comments on Issue #3 (New SC Change of Content)
proactively for tomorrow's.

First was James comment in github about an exception for advertisements and
content not associated with the primary purpose of the page. I agree and
have added the exception.
Second, COGA issue #54 has some overlap and Ive added a requirement for
visual notification for actions that has a success or failure response.
This is a proposal to combine #53 and #3. If others agree then we can begin
to amalgamate the Understanding from the two. This is a first stab at
combining them.


(Change of Content): Programmatic notification and is provided for each
change in content that indicates an action was taken or that conveys
information, unless one or more of the following is true:
1. The change immediately follows the control that triggered it, in the
programmatic reading order. 2. There is an accessibility supported
programmatic relationship between the new content and the user activated
control that triggers it. 3. The user has been advised of the behavior
before using the component. 4. There are more than 5 notifications per
minute. 5. The content is not related to the primary purpose of the page
AND is not a result of a user action. 6. If the user action results in
success or failure response, then a visual indication AND a programmatic
notification are provided.
