There are two elementsthat can be used:

I want to stress that something is <em>really</em> important...: the
word "really" would be rendered in italics by most browsers
<strong>Note:</strong> The following material is...: The word "Note" and
the colon will usually be rendered in boldface.

CSS can be used to change the visual presentation if the defaults don't
suit you.  

Some screen readers can be set to change pitch or voice when they
encounter bolded or italicized text.
